Mattie Foundation Frequently Asked Questions

The Mattie Foundation's mission is to assist those who work in the Arizona hospitality industry that do not have medical insurance or have a high deductible/co-pay and need some financial help to get minimal healthcare services such as doctor's visits, lab fees and prescriptions.

The Mattie Foundation founded in memory of Matt Metzer who touched the lives of many people through his kindness, generosity and work in the downtown Scottsdale hospitality industry.  Matt would greet everyone with a big hug and was willing to go the extra mile for his customers, friends and co-workers.

The Mattie Foundation was founded to assist those employed in the hospitality industry in Arizona with the reimbursement of expenses for the treatment of an illness or medical condition provided by a licensed healthcare provider.
The following criteria will be used in determining if an applicant is eligible to receive financial assistance from the foundation:

1) Verifiable employment by a hospitality company that provides food, beverage or lodging in Arizona.

2) Individual is not covered by personal, employer provided or public health insurance such as Medicare, Medicaid or other governmental program Or Individual who has health insurance with a deductible of $5,000 or more per year.

Applicants are required to fill out an application for assistance and provide verifiable receipts for doctor's visits, prescriptions or lab fees to be considered for reimbursement.  Expenses without receipts will not be considered as eligible and will not be reimbursed.
